in the capacity of  Next of Kin

of the deceased (full name, date of birth, passport number)

 

hereby appoints and authorizes either

 

Wilfried Verbruggen (Belgian, passport no EG070926) or

Hu Xiaofeng (Chinese, passport no G21775585, ID 110105196912189433) or

an individual appointed by one of the aforementioned proxy holders

to deal with all formalities in China related to the repatriation of:

-          the deceased  in coffin / in urn (delete what is not applicable)

-          the personal effects (delete if not applicable)

to (name of country)

I also authorize the Chinese relevant authorities to autopsy the human remains whenever there is a legal ground as per Chinese law.
